Our office is open weekly Monday through Thursday from 8:00 AM to 5:00 PM.

New Patient Forms
Completing the new patient forms is a two-step process:
- Please click here to download and print the patient forms;
- Complete the forms and please bring them with you at your scheduled appointment.
Appointment Instructions
Please arrive at your appointment 15 minutes before the scheduled time. We are making individualized appointments to offer the dental care that you are entitled to, so your punctuality contributes to a significant degree to keeping our prices low and the quality of our services high. Thank You!
We are confirming all appointments 48 hours in advance. In case we cannot reach you directly and we leave a message or contact you by email, we request you to callback or email us at office@nasteadds.com to confirm and preserve your appointment.

Important Notes:
- Please notify the office prior to the visit if you have special medical conditions or concerns. Some medical conditions may require premedication for some procedures.
- A parent or guardian must accompany to the consultation every patient under the age of 18.
Dental Emergencies
In case of a dental emergency during off hours, you have two options:
- You can either call the office at 703.569.9494 and leave us a message, and we will return your call on the morning of the next business day to schedule an emergency appointment;
- Or, click here for instructions on how to be contacted by the Doctor for a telephone emergency consultation.
Cancellations
In case you need to cancel and reschedule the appointment, we request thay you give us 48 business hours advanced notice to allow us to rearrange our schedule with another patient needing our services. Please be aware that there is a $65 fee for broken appointments or late cancellations within 48 business hours before the appointment.
